29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Building Interior: An employee was exposed to a fall hazard of approximately 25 feet while standing on the mid rail of a scissor lift while installing bolts on a pre-fabricated structure,  on or about 11/18/2019.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) Building Interior: Employees working from a scissor lift  scaffold system were not trained in the recognition and avoidance of hazards associated with scissor lift usage, on or about 11/19/2019.
