Test voltages and the maximum intervals between tests shall be in accordance with Table I-4 and Table I-5.    Electrically rated gloves used to reset contactors had not been tested at intervals required exposing employees to electrical shock hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.       Note: Personal protective equipment requirements are contained in subpart I of this part.  On or about December 17, 2019, an employee was observed on machine #130 offset printing press in the Placemat Department not utilizing electrical safe work practices.  The employee was wearing a short sleeve work shirt and polyester hair net which was not appropriate for exposure to live electrical.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    (a)  In the Legacy Department, the #29 CG Bretting Mfg Co (S/N:  2798-80) had an unguarded inrunning nip point of the unwind machine for the roll guard.    (b)  In the Legacy Department, the #55 CG Bretting Mfg Co (4 Color Letterpress Printer, 10" Repeat, S/N:  2841-80) had the nip guard located by the embossing rollers in the lowered position exposing employees to pinch points.     (c)  In the Legacy Department, the # 50 CG Bretting Mfg Co had unguarded inrunning nip points on the top ink deck exposing employees to pinch points.    (d)  In the Legacy Department, the #7 High Speed CG Bretting Mfg Co (LCI Napkin Folder, S/N:  6327-16F) had inrunning nip points by the folding section exposing employees to caught in hazards.    (e)  In the Legacy Department, the #7 High Speed CG Bretting Mfg Co (LCI Napkin Folder, S/N:  6327-16F) had inrunning nip points by the Cam 0881 Fi Folder (drive side) and folder pull roll wrap up (drive side) exposing employees to caught in hazards.    (f) In the Legacy Department, the High Speed #5 had a guard which was easily removed allowing access to rollers during operation exposing employees to pinch hazards.    (g)  In the Fluted Department, the Fluted Bagger had a damaged interlocked door (Schmeadsal Safety Component) exposing employees to moving parts.     (h)  By Fashion Point, the Baler by Balemaster had unguarded doors which allowed access to moving parts exposing employees to struck by hazards.    (i)  In the Lace Department, #330 lace machine had unguarded inrunning nip points on the green belt exposing employees to caught in hazards.    (j)  In the Lace Department, #330 lace machine had unguarded inrunning nip points on the spaghetti belts exposing employees to caught in hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    (a)  In Building 5, Prism High Speed Cutter (Model:  QZK-920-1, Size:  92, S/N:  986,1039) had an unguarded clamp and the light curtain was positioned approximately 6 inches above the table which allowed access under the presence sensing guard, exposing employees to struck-by and amputation hazards.

29 CFR 1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a)  On or about August 28, 2019, an employee was exposed to machine hazards associated with moving parts when they engaged in threading activities on the #72 CG Bretting (S/N:  3551-86, Single web 10" cocktail folder).  The employer did not ensure that employees implemented energy control application steps prior to allowing them to thread the machine.  The machine was not deenergized and an one-hand jog button was utilized while the employee engaging in the threading activity.    (b)  On or about December 17, 2019, in Building 5, an employee was exposed to machine hazards associated with moving parts when they engaged in clearing jam activities on the #330 Paper Converting Mach Co.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to unjam the machine.  The machine was not deenergized prior to the employee engaging in the servicing/maintenance activity.      (c)  On or about January 10, 2020, in the Legacy Department,  an employee was exposed to machine hazards associated with moving parts when they engaged in threading activities on the #16 CG Bretting Legacy Napkin Machine.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to thread the machine.  The machine was de-energized, however, energy isolating devices were not applied prior to the employee engaging in the threading activity.      As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.    The Hoffmaster Group Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1200091, citation number 1, item number 2 and was affirmed as a final order on April 28, 2017, with respect to a workplace located at 255 Spring St., Clintonville, Wisconsin.
