OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from hyperthermia: (a) Facility, 2362 Golden Mile Highway, Pittsburgh, PA: On or about November 21, 2019 the door handle inside the "ice cream" freezer was missing and not capable of being opened when the door has been closed thereby impeding escape from the freezer.

29 CFR 1910.305(b)(1)(iii)(F): Where cable is used, each cable was not secured to the cabinet, cutout box, or meter socket enclosure. However, where cable with an entirely nonmetallic sheath enters the top of a surface-mounted enclosure through one or more nonflexible raceways not less than 457 mm (18 in.) or more than 3.05 m (10.0 ft) in length, the cable need not be secured to the cabinet, box, or enclosure provided that the raceway is fastened at its outer end and at other points as necessary: (a) Facility, 2362 Golden Mile Highway, Pittsburgh, PA: On or about November 21, 2019 the cable running into the box to control the power for the freezer did not have a raceway that was fastened at its outer end exposing the conductors.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Facility, 2362 Golden Mile Highway, Pittsburgh, PA: On or about November 21, 2019 the 277v flexible cord located on the "ice cream" freezer did not have strain relief so that there would be no tension transmitted to joints or terminal screws.
