29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for hazards associated with powered industrial vehicles and lack of operator training.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about November 21, 2019, at a worksite located at 330 North Meridian road, Youngstown, OH 44509, during commercial building manufacturing activities the employer did not ensure the operator of Toyota powered industrial vehicle, Model 7FGCSU20, type LP and powered industrial vehicle, Yale Veracitor, Model 80VX, Type LP were competent and trained. The operators had not received any training or certification from the employer prior to operation.
