29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.    a) Worksite, 1280 Meadow Lane, Southampton, New York, residential building: An employee working from a JLG Mobile Elevating Work Platform 660SJ (17862) was exposed to falling approximately 15 feet while installing windows, on or about 11/19/19.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.454(b)(2): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question.    a) Worksite, 1280 Meadow Lane, Southampton, New York, residential building: Employees were not trained by a competent person on the applicable standards or procedures to be followed while working from a JLG Mobile Elevating Work Platform 660SJ (17862), on or about 11/19/19.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
