29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) Southeast corner of the roof: On or about 11/21/2019, a 2 foot diameter circular hole on the walking/working surface was not protected from falling through the hole, from the roof down to the 2nd floor, creating a fall hazard of approximately 35 feet high.
