29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): On or about November 20, 2019, at 2565 Apple Valley Road, Brookhaven, Georgia: a) Employees performing brick masonry work from a tubular welded frame scaffold (scaffold b) were exposed to fall hazards of approximately 21-feet when they worked from the 3rd buck work platform that was not fully planked and decked. b) Employees performing brick masonry work from a tubular welded frame scaffold (scaffold c) were exposed to fall hazards of approximately 14-feet when they worked from the 2nd buck work platform that was not fully planked and decked.

On or about November 20, 2019, at 2565 Apple Valley Road, Brookhaven, Georgia: a) An employee performing brick masonry work from the 2nd buck of a tubular welded frame scaffold system (scaffold b) that was approximately 28-feet long and 28-feet high was exposed to a fall hazard of approximately 14-feet when the scaffold system was not attached at any point to the building. b) Two employees performing brick masonry work from the 3rd buck of a tubular welded frame scaffold system (scaffold b) that was approximately 28-feet long and 28-feet high was exposed to a fall hazard of approximately 21-feet when the scaffold system was not attached at any point to the building.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about November 20, 2019, at 2565 Apple Valley Road, Brookhaven, Georgia: Employees performing brick masonry work from a tubular welded frame scaffold (scaffold b) were exposed to fall hazards of approximately 21-feet and greater when they had to climb the frame of the scaffold to get to the 3rd and 4th buck work platforms of the scaffold because the ladder was installed from the ground to approximately 20-feet up the scaffold.
