29 CFR 1910.28(b)(6)(i): The employer did not ensure each employee less than 4 feet (1.2 m) above dangerous equipment was protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system when the equipment was not covered or guarded to eliminate the hazard:   a) On or about 11/15/2019, at WNY BICS located at 4284 Walden Avenue, Lancaster, New York; a Cascades employee was fatally injured when he fell into a conveyor belt area while dumping aluminum cans onto a moving conveyor belt located in a shallow pit with slopped sides converging into the conveyor belt. Employees who operate the conveyor belt are in close proximity to the slopped sides that converge into the conveyor belt and are exposed to falling into the conveyor belt while its running. The conveyor belt was not covered or guarded to prevent employees from stepping or falling onto the moving belt.   ABATEMENT DOCUMENTATION REQUIRED
