On a worksite located at the Penthouse 2316 E. Meyer Boulevard, Kansas City, Missouri: Employees were exposed to electrical hazards. Two extension cords were run through a closed door from the Penthouse to power the Dewalt saw and a heat gun.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On a worksite located at the Penthouse 2316 E. Meyer Boulevard, Kansas City, Missouri: Employees were exposed to electrical hazards. The extension cord powering the DeWalt saw did not have adequate strain relief.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On a worksite located at the Penthouse 2316 E. Meyer Boulevard, Kansas City, Missouri: Employees were exposed to fall hazards. Roofers were observed working at the edge of the roof working at height. Roofers were exposed to a fall distance of 25 feet eave to ground without the use of fall protection.
