No fall protection plan was in place and fall protection devices were not in place.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) 3D Custom Builders, LLC located at 1216 W High Ave. New Philadelphia, OH 44663: On and preceding November 22, 2019, the employer did not ensure that employees were protected from falling through holes more than 6 feet above the lower level by a PFAS, guardrails or covers. Throughout the process of installing a parapet wall and other framing pieces 15 feet above ground level, employees were working around floor holes created by several gaps in the decking panels. The gaps between panels least dimension measured between 6" and 30" which exposed employees to falls up to 15 feet.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) 3D Custom Builders, LLC located at 1216 W High Ave. New Philadelphia, OH 44663: On and preceding November 22, 2019, the employer did not ensure that employees were protected from tripping in or stepping into or through holes by covers. Throughout the process of constructing walls, setting trusses and framing in a commercial building, employees were working around floor holes located around two structural support beams on the ground level of the building. The following condition exposed employees to trips from floor holes measuring 9-24" wide and 6" deep.
