29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. Site - Employees performing roofing work from a flat roof were not protected from falls of approximately 15 feet when fall protection equipment was not being used.  b. Site - Employees performing roofing work from a sloped roof were not protected from falls of approximately 18 feet when fall protection equipment was not being used.  c. Site - Employees performing roofing work on the sloped roof of a detached garage were not protected from falls of approximately 8 feet when fall protection equipment was not being used.   Jose Guaman and his company, JVGM Construction Corp., was previously cited for a REPEAT violation of this occupational safety and health standard, 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 1416846, citation number 002, item number 002, and was affirmed as a final order on October 16, 2019, with respect to a workplace located at 23 Beacon St. Concord, NH 03301.  Jose Guaman and his company, JVGM Construction Corp., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29CFR 1926.501(b)(11), which was contained in OSHA inspection number 1355271, citation number 001, item number 002, and was affirmed as a final order on January 14, 2019, with respect to a workplace located at 301 Turnpike Rd. 29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally:  Site - Employees performing roofing work on the detached garage were not protected from falls of approximately 8 feet when an aluminum extension ladder used to gain access to and from the roof was placed against the gable end of the roof and the ladders side rails were not supported equally due to the slope of the roof.
