On or about November 21, 2019, at the flat roof, the employer did not ensure that employees were in use of fall protection while performing roofing work activities from the flat roof of a building. This exposed the employees to falls greater than six (6) feet to a lower level.

The employer did not ensure that each employee had access to labels on containers of asbestos and safety data sheets, and was trained in accordance with the provisions of the Hazard Communication Standard and paragraphs (k)(9) and (10) of this Section. The employer did not provide information on at least the following hazards: Cancer and lung effects. a. On or about November 21, 2019, for cleanup work at the flat roof, the employer did not ensure that asbestos was included in a written hazard communication program. Employees removed gravel and debris from the asbestos containing flat roof and had the potential for contacting asbestos containing roofing material. This exposed the employees to health hazards.
