OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in hazards: (a) On or about 11/13/2019 - Line 1 Palletizer, guarding was not adequate to protect the employees from being caught-in the in-going nip point between the ingoing mesh conveyor belt and the Palletizer rollers.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: (a) On or about 11/13/2019 - Line 1 Palletizer, employees were exposed to caught-in hazards when the sprocket wheel and chain was not enclosed and the light curtain safety device that had been installed to stop the equipment if an employee reached into the area was not adequate.
