29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   A)     MPW Environmental Services, Inc., conducting work at 4701 Bay Shore Road, Oregon, OH 43616: On or about November 15, 2019, the employer did not ensure that employees were protected by adequate personal protective equipment such as, but not limited to, a metatarsal guard, gaiter, or a combination shin/metatarsal guard while employees were using the NLB Corporation High Pressure Water Jetting System to remove hardened fly ash at the Pug Mill of the Bay Shore Power Plant. Employees were exposed to approximately 16,000 psi water stream.
