29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On 11/21/2019, employees of S&S Utility Contracting Company were working in a trench located at or near 1297 Feise Road, OFallon, MO, 63368. The employer did not ensure that benching, sloping, shoring or other means was used to protect employees from cave-in, engulfment, or struck-by hazards. The maximum allowable slope of 45 degrees from horizontal for a type B soil was exceeded. S&S Utility Contracting Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a), which was contained in OSHA inspection number 1333927, citation number 1, item number 2 and was affirmed as a final order on 11/20/2018, with respect to a workplace located at 54 West Industrial Drive, OFallon, Missouri.
