29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a. On or about November 22, 2019, located in the Mold Press Department, the employer did not conduct periodic inspections of the energy control procedures for the hydraulic mold presses, thereby exposing the employees to the hazards associated with service and maintenance activities.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets.   a. On or about November 22, 2019, located on the back of press 201, there was an open receptacle box with wire nuts on the ends of its conductors, thereby exposing the employees to an electrical shock hazard of approximately 120 vac.  b. On or about November 22, 2019, located on the back of press 202, there was an open receptacle box with wire nuts on the ends of its conductors, thereby exposing the employees to an electrical shock hazard of approximately 120 vac.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  a. On or about November 22, 2019, located outside the front of Press 201, there was a slitter table with an ITS (Innovative Tooling Solutions) electrical control box with a flexible cord plug end without a grounding pin, thereby exposing employees to fire and electrical shock hazards up to 120 VAC.  b. On or about November 22, 2019, located behind Press 209, there was a floor fan supplied with an extension cord with a flexible cord plug end without a grounding pin, thereby exposing employees to fire and electrical shock hazards up to 120 VAC.  c. On or about November 22, 2019, located at Press 217, there was a floor fan supplied with an extension cord with its cord plug end without a grounding pin, thereby exposing employees to fire and electrical shock hazards up to 120 VAC.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  a. On or about November 22, 2019, located outside the front of Press 201, there was a slitter table with an ITS (Innovative Tooling Solutions) electrical control box with a flexible cord plug end with its cord jacket pulled back leaving the cord and plug without strain relief provided, thereby exposing employees to fire and electrical shock hazards up to 120 VAC.  b. On or about November 22, 2019, located outside of Press 217, there was a Dayton floor fan that did not have strain relief provided where its flexible cord entered the fans motor housing, thereby exposing employees to fire and electrical shock hazards up to 120 VAC.

29 CFR 1910.137(c)(2)(viii): Electrical protective equipment shall be subjected to periodic electrical tests. Test voltages and the maximum intervals between tests shall be in accordance with Table I-4 and Table I-5.  a. On or about November 22, 2019, employees performed electrical work with one set of rubber insulated gloves that were dated with their last electrical test in February 2014 and another set tested on March 23, 2018. The employer did not ensure the rubber insulated gloves were tested at least every six months.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  a. On or about November 22, 2019, located along the aisle next to the SMC Department, there were two fire extinguishers that were mounted along the wall but were not identified by any means to make them readily accessible to employees.  b. On or about November 22, 2019, located behind Press 209, an area was identified as for a fire extinguisher but no fire extinguisher had been mounted so the fire extinguisher would be readily accessible to employees.

On or about November 22, 2019, located outside the front of Press 217, there was an extension cord plugged into a floor fan that had damage to its jacket leaving only the conductor's internal insulation to protect the employees from electrical shock.
