29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a. 3700 Palisades Ave Union City, NJ Employee (s) performing construction operations from a fabricated frame scaffold were using the crossbraces to access the upper working areas of the fabricated frame scaffold, exposing employees to fall hazards of approximately 25 feet. Violations occurred on or about 11/13/2019

Workers performed restoration activities from a tubular frame scaffold at heights of 25 feet or more without fall protection as the scaffold was missing multiple sets of guardrails. Violations occurred on or about 11/13/2019
