On or about November 25, 2019, during the installation of roofing material at the job site located at 7582 Rolling Green Ave NE, Massillon, Ohio 44647; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about November 25, 2019, during the installation of roofing material at a job site located at 7582 Rolling Green Ave Ne, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 11.6 feet by utilizing a fall protection system. The M-M Miller Roofing, LTD./ M&M Miller Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1111640, citation number 01, item number 01 and was affirmed as a final order on 11/21/2016, with respect to a workplace located at 7663 Rolling Green Ave NW, Massillon, Ohio 44646.
