29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: a) On or about November 20, 2019 and at times prior thereto; 12 employees removing asphalt and wood shingles from a 10:12 pitch roof were not protected from 10-18 foot falls. b) On or about November 20, 2019 and at times prior thereto; 12 employees removing shingles from a 10:12 pitch roof accessed an extension ladder that was not extended three feet to the landing surface, or secured to prevent the ladder from shifting in gravel landscaping rock. Note: Abatement certification and documentation are required for this item.
