The employees were exposed to a fall hazard.

29 CFR 1910.28(b)(6)(i): The employer must ensure each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: JOB SITE: 2388 N. US Hwy 67, Florissant, MO 63033, on or about 11/26/2019, workers operating the SHRED-TECH MDS Series GT Shredder, were permitted to climb up the bin tunnel into the hopper to clear the shredder, approximately 90 inches above ground level without any form of fall protection. The employer did not ensure each employee was protected from falling into or onto dangerous equipment as required.

US Hwy 67, Florissant, MO 63033, on or about 11/22/2019, an employee who was engaged in unclogging a jam on the SHRED TECH MDS Series GT Shredder was injured and sustained an amputation to the toes. The incident occurred on 11/22/2019, and was not reported to OSHA until 11/25/2019.
