(a) Delphi press robotic cell: On or about November 16, 2019, The employer did not utilize procedures to insure Robotic press cell was deenergized and locked out when employee(s) entered the safety system enclosure to clear a jam on production process equipment. This exposed employee(s) working on or around robotic press cell to point of operation, ingoing nip point, rotating parts, struck by, and/or crushing hazards. (b)Delphi press robotic cell: On or about November 25, 2019, The employer did not utilize procedures toinsure Robotic press cell was deenergized and locked out when employee(s) entered and/or reached into the safety system enclosure and related machines to work on a press that had been continually jamming during the production process. This exposed employee(s) working on, or around the robotic press cell to point of operation, ingoing nip point, rotating parts, struck by, and/or crushing hazards.

1910.212(a)(1): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and crushing hazards. (a) Delphi press robotic cell: On or about November 16, 2019, the employer did not insure guarding was properly implemented on cell safety system exposing employee(s) working on or around robotic press cell point of operation, ingoing nip point, rotating parts, struck by, and/or crushing hazards that were created when employee entered safety system enclosure to clear a machine jam. The employee entered the robotic cell without removing the enclosure gate key interlock. (b) Delphi press robotic cell: On or about November 25, 2019, the employer did not insure guarding was properly implemented on cell safety system exposing employee(s) working on or around robotic press cell point of operation, ingoing nip point, rotating parts, struck by, and/or crushing hazards that were created when employee(s) entered safety system enclosure to work on a machine jam. The employee(s) entered the robotic cell without removing the enclosure gate key interlock, and without insuring control panels were disabled.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Delphi robotic cell: On or about November 16, 2019, employees inside the cell enclosure were not protected from the point of operation of the power molding press when reaching into the point of operation to clear a jam. This exposed the employee to an amputation injury.
