29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    (a) South side of the building:  On or about 11/25/2019, the employer allowed employees to work on the top level of the tubular welded frame scaffold, while the top level was not fully planked between the uprights, creating a fall hazard of approximately 8 1/2 feet.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:  (a) South side of the building:  On or about 11/25/2019, employees performed work from the tubular welded frame scaffold, while some of the legs of the scaffold rested on a wood plank which was stacked on a rock and a concrete block, exposing the employees to a fall hazard of approximately 8 1/2 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access:    (a) South side of the building:  On or about 11/25/2019, an employee was observed climbing the scaffold frame to access the lower level of the scaffold, approximately 4 1/2 feet high.
