29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On and about November 16, 2019, an employee conducting cleaning operations on a glue roller assembly on a Nordmeccanica Super Simplex SL 1500 Laminator, Serial Number DW1957 without covering the following actions required by 29 CFR 1910.147(d)(1)-(6): -the employees did not have to apply energy isolation devices or the equipment was not operated in such a manner to isolate the machine from all energy source(s) [(d)(3)], -the employees did not apply isolation devices by each authorized employee [(d)(4)], -the employees did not verify that isolation and Deenergization of the machine or equipment had been accomplished [(d)(6)].
