29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) All Wall Construction, LLC at the worksite of 941 Interstate Drive in Findlay, Ohio; On or about November 26, 2019 the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during jobsite operations.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  a) All Wall Construction, LLC at the worksite of 941 Interstate Drive in Findlay, Ohio; On or about November 26, 2019 the employer did not ensure employees performing work from an aerial lift system were properly trained by a competent person to recognize and avoid hazards specifically associated with this type of aerial lift, such as fall hazards. Employees were exposed to hazards associated with falls of approximately 30 feet while operating the JLG, #140629, and securing sheeting to the roof of the commercial structure.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) All Wall Construction, LLC at the worksite of 941 Interstate Drive in Findlay, Ohio; On or about Novemebr 26, 2019 the employer did not ensure employees wore a body belt and lanyard attached to the boom or basket when working from an aerial lift. Employees were exposed to hazards associated with falls of approximately 30 feet while operating the JLG, #140629, to move materials.
