29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set:   On or about 11/25/2019, International Paper, located at 5300 Bircher Blvd., St. Louis, MO, instructed their clamp powered industrial truck (PIT) operators to park their PITs with the clamp elevated off of the floor.  The employer stated this was to protect the life of the pins within the paper roll clamping mechanism.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:  On or about 11/23/2019, International Paper, located at 5300 Bircher Blvd., St. Louis, MO, the clamp truck (PIT) operator was backing his clamp truck (PIT) backwards without looking in the direction of travel.  This involved a struck-by incident when the PIT struck another employee.  This incident put the victim of the incident in the hospital.
