29 CFR 1910.67(c)(2)(v): The employer did not ensure a personal fall arrest or travel restraint system that meets the requirements in subpart I was worn and attached to the boom or basket when working from an aerial lift: On or about November 26, 2019, an employee was exposed to a fall hazard of approximately 15 feet while in the bucket of a vehicle mounted aerial lift replacing light bulbs and the employer did not ensure suitable fall protection was being used.
