29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Job site, 7601 McKnight Road, Pittsburgh, PA: On or about November 20, 2019, during construction activities, the employer did not ensure that floor holes were provided with covers, exposing employees to the hazard of stepping into or tripping in an opening at least approximately 124 inches long, 50 inches wide and 3 inches deep.
