29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about November 26, 2019, the employer failed to develop and implement a written hazardous energy control program.  Employees are tasked with clearing jams, servicing and repair of various machinery, to include but not limited to the extrusion lines.  Employees are exposed to amputation hazards as well as energy sources to include electrical and pneumatic.

29 CFR 1910.147(c)(2)(ii): If an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section shall utilize lockout, unless the employer can demonstrate that the utilization of a tagout system will provide full employee protection as set forth in paragraph (c)(3) of this section:  On or about November 26, 2019, the employer failed to ensure that machines that were capable of being lock and tagged out, such as but not limited to the Extruder Lines 1-12, 14-15.  These lines were not locked and tagged out prior to employees performing tasks, such as but not limited to, clearing jams, correcting line breaks and stringing up lines.  Employees are exposed to amputation hazards as well as energy sources to include electrical and pneumatic.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate:    On or about November 26, 2019, the employer failed to ensure that employees who were tasked with clearing jams, correcting line breaks and stringing up lines on the Extruder Lines (1-12, 14-15) were trained as authorized to perform such tasks which fall within the scope of the control of hazardous energy program as trained and authorized employees.  Employees are exposed to amputation hazards as well as energy sources to include electrical and pneumatic.
