29 CFR 1926.451(d)(5)(iii): Opposing angle tiebacks were not installed and not secured to a structurally sound point of anchorage on the building or structure. Sound points of anchorage include structural members, but do not include standpipes, vents, other piping systems, or electrical conduit.     Location:  a) Suspended scaffold on the northwest side of the jobsite:     On or about 11/26/2019, an employee was repairing the exterior wall of the building by working on a suspended scaffold. The suspended scaffold tiebacks were installed and secured to guardrails piping and electrical equipment mounting pipes. They were not structurally sound point of anchorage.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.      Location:  a) Sidewalk shed on the southwest side of the jobsite:    On or about 11/26/2019, employees climbed fences and scaffold bracing as a mean of access to access the sidewalk shed level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Location: a) Sidewalk shed on the southwest side of the jobsite:    On or about 11/26/2019, employees were accessing the suspended scaffold from the sidewalk shed, and there was an opening next to the access point that exposed employees to falling approximately 15 feet to the ground level. The employees were not protected from fall by the use of personal fall arrest systems, or guardrail systems.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed; the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     Location: a) Sidewalk shed on the southwest side of the jobsite:    On or about 11/26/2019, employees used a folded A-frame ladder for access between the lower sidewalk shed and to the upper sidewalk shed. The ladder was not extended at least three feet above the landing being accessed and was not secured at its top.

Location:  a) Sidewalk shed on the southwest side of the jobsite:     On or about 11/26/2019, employees used a folded A-frame ladder for access between the lower sidewalk shed and to the upper sidewalk shed. The ladder was not designed to be climbed while it is folded.
