On or about November 25, 2019, employees were performing roofing work on a low-slope roof. There was approximately a 100 foot section of warning lines that had been erected less than 6 feet from the roof's edge, parallel to the direction of mechanical equipment operation. There was also approximately 50 foot section of warning lines that had been erected less than 10 feet from the roof edge which was perpendicular to the direction of mechanical equipment operation, thereby exposing the employees to a fall hazard of approximately 20 feet.
