Paint Room - The employer did not develop and implement a written respiratory protection program, with workplace-specific procedures addressing the major elements of the program.  An employee was required to wear an air purifying, half mask North 7700-30S respirator during the mixing of the dyes and an air purifying, Honeywell full-face respirator P/N 80802 during the spraying of dyes onto fabrics. 29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a. Paint Room - The employer did not ensure that an employee was medically evaluated prior to fit testing and initial use of her respirators.  The employer required an employee to wear an air purifying, half mask North 7700-30S respirator during the mixing of the dyes.  Also, the employer required this employee to wear an air purifying, Honeywell full-face respirator P/N 80802 during the spraying of dyes onto fabrics.  Observed On or about 10/16/19

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  a. Rear of the Building, ADC American Dryer, Model Number AD81 - The employer did not provide the employees with training in the proper use of portable fire extinguishers. The employer required this employee to use a portable fire extinguisher to put out a small fire.  This employee was exposed to a burn hazard. Observed On or About October 07, 2019
