29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:     On or about November 26, 2019, prior to allowing employees to use the Gehl Telescopic Handler forklift, the employer did not ensure that each operator had completed formal instruction training, practical training and an evaluation.

29 CFR 1910.178(l)(6): The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:    On or about, November 26, 2019, prior to allowing employees to use the Gehl Telescopic Handler forklift, the employer did not document the required certification for each operator which includes; the operators name, date of training, date of evaluation and/or the name of the person conducting the evaluation.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and  the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access:      On or about November 26, 2019, an employee was exposed to a fall hazard of approximately 13 feet while using a ladder to gain access to a platform that was attached to the forks of a Gehl Telescopic Handler and the rails did not extend three feet above the platform.
