29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 2249 SW Wall Street, Blue Springs, Missouri: Employees were using a pneumatic nail gun while engaged in roofing activities but were not using proper eye or face protection to protect them flying debris.  This hazard was observed on November 25, 2019.     Gustavo Montoya was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102 (a)(1) Eye/Face Protection, which was contained in OSHA inspection number 1327256, citation number 1, item number 2 and was affirmed as a final order on 12/20/2018, with respect to a workplace located at 617 2nd Street Odessa, Missouri.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At the residential construction site located at 2249 SW Wall Street, Blue Springs, Missouri: Employees were engaged in roofing activities from elevated heights without the use of fall protection, thus exposing employees to fall hazards. The height of the roof was 8.8 feet from the eave of the residence to the ground below on the east side of the structure.  This hazard was observed on November 25, 2019, and November 27, 2019.    The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1327256, citation number 1, item number 3 and was affirmed as a final order on 12/20/19, with respect to a workplace located at 617 South 2nd Street, Odessa, Missouri.
