Sioux Court, Independence, Missouri: Employees were exposed to fall hazards greater than 6 feet. 1. Employees were working from the roof deck without the use of a fall protection system. 2. Employees were working from a trailer installing new roofing material without the use of a fall protection system.

Sioux Court, Independence, Missouri: Employees were using a ladder to access and egress a roof when the ladder was not extended at least 3 feet above the landing surface exposing employees to fall hazards greater than six feet.
