29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    a) World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL: on or about November 13, 2019, employees assigned to operate electric pallet jacks (powered industrial vehicles) for order picking did not receive the initial operator training and evaluation in safe operation prior to being released for regular work duties. Employees operating equipment were exposed to struck-by hazard likely to result in serious injury.    b) World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL: on or about November 13, 2019, the employer failed to establish electric pallet jack operator training consisting of formal instruction, practical training, and evaluation of the operator's performance in the workplace. Employees assigned to operate equipment for order picking did not receive the initial operator training and evaluation in safe operation prior to being released for regular work duties. Employees operating equipment were exposed to struck-by hazard likely to result in serious injury.     c) World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL - on or about November 13, 2019, the employer failed to provide electric pallet jack operators with initial training in safe operation of the vehicles while using the voice recognition headsets required for regular order picking tasks. Employees were exposed to struck-by hazards likely to result in serious injury.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(2)(iii): All operator training and evaluation shall be conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence:  Worksite at 1510 Cargo Ct., Minooka, IL (World Class Distribution, Inc.): on or about November 13, 2019, the employer failed to provide competent trainers and evaluators with the knowledge, training, and experience in truck and workplace specific operations. Electric pallet jack operators were exposed to struck-by hazards likely to result in serious injury because evaluators did not provide the required initial operator's practical training and evaluation in safe operation.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.    World Class Distribution Inc. worksite at 1510 Cargo Ct. in Minooka, IL 60447: on or about November 27, 2019, the employer failed to provide within four business hours the requested OSHA 300 Log of Work-Related Injuries and Illnesses for 2019 year-to-date, and OSHA 301 Injury and Illness Incident Report(s) for all recordable injuries in 2019 year-to-date.    There is no abatement certification or documentation required for this item.
