Employees operating equipment were exposed to struck-by hazard likely to result in serious injury.    b) World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL: on or about November 13, 2019, the employer failed to establish electric pallet jack operator training consisting of formal instruction, practical training, and evaluation of the operator's performance in the workplace. Employees assigned to operate equipment for order picking did not receive the initial operator training and evaluation in safe operation prior to being released for regular work duties. Employees operating equipment were exposed to struck-by hazard likely to result in serious injury.     c) World Class Distribution, Inc. warehouse at 1510 Cargo Ct., Minooka, IL - on or about November 13, 2019, the employer failed to provide electric pallet jack operators with initial training in safe operation of the vehicles while using the voice recognition headsets required for regular order picking tasks.
