29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) LHJ Remodeling, LLC located at 297 Bruce Court, Westerville, OH 43081:  On and preceding December 2, 2019, employees were removing shingles preparing for a re-roof on the roof of a two story residential home at heights up to 24 feet without fall protection or PPE. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  a) LHJ Remodeling, LLC located at 297 Bruce Court, Westerville, OH 43081:  On and preceding December 2, 2019,  the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were removing/installing shingles on a roof that varied from 10-24 feet with the employees working at different levels. The employees were not utilizing head or fall protection which exposed them to falls up to 24 feet and struck by hazards to the head.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) LHJ Remodeling, LLC located at 297 Bruce Court, Westerville, OH 43081:  On and preceding December 2, 2019,   the employer did not ensure that employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects utilized head protection. Roofers were removing shingles on a roof with their tools and materials on the upper and lower level. The crew of roofers were working at different levels, below the materials on the upper level. The employees were not utilizing proper head protection which exposed the them to struck by hazards to the head.

Roofers were removing shingles on a roof that varied from 10-24 feet near the edge of the roof. The employees were not utilizing proper fall protection which exposed them to falls up to 24 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) LHJ Remodeling, LLC located at 297 Bruce Court, Westerville, OH 43081:  On and preceding December 2, 2019,  the employer did not ensure that the portable ladders used to gain access to the lower and upper portion of the roof extended at least three feet beyond the upper landing that they were using for access.  Two extension ladders used to gain access to the roof between 10-18' above ground level were positioned 12-14 inches beyond the upper landing.  The following condition exposed employees to falls up to 18 feet.

Two extension ladders used to gain access to the roof between 10-18' above ground level were positioned 12-14 inches beyond the upper landing.  The following condition exposed employees to falls up to 18 feet.
