29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s)provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) 5294 Morning Star Drive, Allentown, PA, Worksite, Roof - On or about November 5, 2019, employees were performing residential roofing work, approximately 9 to 18 feet above grade, without utilizing approved fall protection. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 5294 Morning Star Drive, Allentown, PA, Worksite - On or about November 5, 2019, employees were performing residential roofing work while utilizing an extension ladder that was not extended at least three feet above the upper landing surface. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
