Maintenance building rear exit landing: there was no exit sign present indicating that this door led to the outside as an emergency exit.   2. Maintenance building second floor: there was no exit sign present in the area of the top of the stair case leading to the first floor exit.  3. Maintenance building first floor: front stair landing, there was no exit sign above the exit at this location.

The acetylene cylinder was found standing upright unsecured near flammable and combustible material.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Boiler room rear area: On or about December 3, 2019, an air compressor used to augment the sprinkler system pressure was unguarded. There was a two foot by 19 inch opening to the spoked pulley located on the floor. Possible amputations, crush injuries, or avulsions are possible if an employees hand would get caught the compressor.

Main Building Laundry: A laundry rack of unclaimed clothing was stored in front of 120/208 volt panel.   2. Main building boiler room, front: A long cardboard box and a long metal pipe were stored in front of the I-T-E 120/208 volt electric panel.   3. Maintenance building second floor:  A plastic bin was stored in front of the load center panel.
