29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. (a) On or about December 3, 2019, a Hyster Model S120XLS LP powered forklift was equipped with approximately 8 foot long fork extensions that were not approved for use by the forklift manufacturer. The attachments were not labeled on the nameplate as required and they were not provided with capacity, operation, and maintenance instruction plates. (b) On or about December 3, 2019, a Heli Model CPYD25C-TY5 LP powered forklift was equipped with approximate 6 foot long for extensions that were not approved for use by the forklift manufacturer. The attachments were not labeled on the nameplate as required and they were not provided with capacity, operation, and maintenance instruction plates.
