29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about 11/19/19 at a work site located at 410 N. Main Street, Jamestown, New York; employees replacing roofing were using ropes with rope grabs attached to their harnesses. An employee did not have his rope grabs adjusted or rigged to prevent free falls more than 6 feet and fell 12 feet to a lower roof. NO ABATEMENT CERTIFICATION REQUIRED

Main Street, Jamestown, New York; employees replacing roofing were using ropes with rope grabs attached to their harnesses. An employee was not trained to properly use and adjust his rope and fell 12 feet to a lower roof due to excess slack. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) On or about 11/19/19 at a work site located at 410 N. Main Street, Jamestown, New York; employees were replacing a roof on a church when a employee fell approximately 12 feet and was admitted to the hospital for approximately 10 days. The hospitalization was not reported by the employer, but reported by an anonymous source on 12/4/19. NO ABATEMENT CERTIFICATION REQUIRED
