29 CFR 1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a)  On December 4, 2019,  employees performing roofing work were exposed to electrical hazards while working in close proximity to a 120/240 volt power line.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a)  On December 4, 2019, employees were exposed to fall hazards of up to approximately 9 feet while performing roofing work to an existing home.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  On December 4, 2019, the employer provided an aluminum extension ladder used by employees to gain access to the roof and did not have the side rails extended at least 3 feet above the surface.
