29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: During roof installation work at the SSM Hospital at 620 E. Monroe, Mexico, MO, on December 4, 2019 no fall protection system, according to the provisions of this standard, were implemented. No safety monitor was designated, or any other fall protection, such as the use of fall protection harnesses when exposed to a fall including retrieving roofing materials to and from the canopy roof on the south side of the main building roof. Employees were exposed to a 40 ft. fall hazard.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: During roof installation work at the SSM Hospital at 620 E. Monroe, Mexico, MO, on December 4, 2019 a 5 gal bucket was used as a step up/down between the main roof and the canopy roof to the south, instead of a ladder or stairway. A four foot distance existed between the two levels.
