29 CFR 1910.178(a)(4):   Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  a.    On or about November 22, 2019, an employee was elevated in Aisle 4B area on the forks of a RR6000 Series Crown High-Low electric motor narrow aisle forklift. The employee was exposed to a fall hazard of approximately 9 feet while lifted up to the second bay of the warehouse storage shelving in the area to obtain product for a customer.  b.    On or about November 26, 2019, an employee was elevated in the Freezer Area while standing on the forks of a RR6000 Series Crown High-Low electric motor narrow aisle forklift. The employee was exposed to a fall hazard of approximately 9 feet while lifted up to the second bay of the warehouse storage shelving in order to obtain product for a customer.  c.    On or about November 26, 2019, an employee was elevated in the Freezer Area while standing on a blue pallet (chap) lifted by the forks of a RR6000 Series Crown High-Low electric motor narrow aisle forklift. The employee was exposed to a fall hazard of approximately 9 feet while lifted up to the second bay of the warehouse storage shelving in order to obtain product for a customer.

29 CFR 1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  a. On or about December 9, 2019, there were three 120/208 Volt electrical panels in the Manager's office that had materials stored within the required working space. The storage of fans, boxes, and other equipment was on and directly under the electrical panels blocking them from being safely accessed.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  a. On or about December 9, 2019 and at times prior, there was a 460-Volt damaged electrical box at the control station for the Compactor in the Receiving Department, which exposed employees to an electrical shock hazard. The electrical box was at waist height and the door was not latched close and live electrical parts were exposed.
