29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about 12/04/2019, at 3651 W 108 Street Hialeah, FL, an employee was not trained as a competent person in the recognition and avoidance of unsafe conditions while having other employees working in the proximity of an excavator performing trenching work. Zahlene Enterprises, Inc. was previously cited for a serious violation of this standard occupational safety and health standard or its equivalent standard 29 CFR 1926.21(b), which was contained in OSHA inspection number 1414211, citation number 01, item number 001, and was affirmed as a final order on 11/20/2019, with respect to a workplace located at the intersection of North Michigan Ave. and Alton Road, Miami Beach, FL.
