On or about December 03, 2019, at the above address, an employee was required to operate a powered industrial vehicle and had not been provided with training, including but not limited to formal instruction, practical training and an evaluation of the operators performance, thereby exposing employees to struck by and crushing hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. On or about December 3, 2019, at the above addressed jobsite, employees performing commercial roofing operations, seventeen (17) feet above the ground, were not protected from falls, thereby exposing employees to fall hazards. J & F Chiattello Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1172917, citation number 1, item number 1 and was affirmed as a final order on November 01, 2016, with respect to a workplace located at 1924 Broadway Ave., Blue Island, IL.
