29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about Novemer 14, 2019 and times prior, employees changed the blades on the 480V RMS rebar sheers with multiple energy sources such as 480V electric and 90psi pneumatic. The employer had not developed, documented or utilized specific energy control procedures for the equipment, during maintenance activities.. The BRG Holdings dba Trinity Rebar was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number 1392971, citation number 1, item number 1 and was affirmed as a final order on August 23, 2019, with respect to a workplace located at 1020 Winfield Rd, Winfield, WV.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Tabletop Rebar Bender: On or about December 5, 2019 and times prior, employees used an KBR Tabletop rebar bender to bend rebar. The rebar bender was not adequately guarded to prevent employee exposure to the moving parts. The BRG Holdings dba Trinity Rebar was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1392971, citation number 1, item number 2 and was affirmed as a final order on August 23, 2019, with respect to a workplace located at 1020 Winfield Rd, Winfield, WV.
