29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  On or about December 04, 2019, at the jobsite located 210 Butternut Avenue, Hannibal, Missouri, the employer did not ensure that the LULL Model 644B-42, Rough Terrain Telehandler Forklift, Serial Number 00Z24N22-1329 was designed to support a pallet used as a work platform.  In the alternative,   29 CFR 1926.453(a)(1):  The Aerial lift was not designed or constructed in accordance with the applicable requirements of the American National Standard for "Vehicle Mounted Elevating and Rotating Work Platforms," A92.2-1969:  The employer did not ensure that the platform used by a worker and supported by the LULL Model 644B-42, Rough Terrain Telehandler Forklift, Serial Number 00Z24N22-1329, was designed to meet the requirements of ANSI A92.2-1969.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:Note:  If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly.   On or about December 04, 2019, at the jobsite located at 210 Butternut Avenue, Hannibal, Missouri, a modification was made to a Lull Model 644B-42, Rough Terrain Telehandler Forklift, Serial Number 00Z24N22-1329, by attaching a pallet for use as a platform for washing brick on a residential dwelling.  The modification was not approved by the manufacturer.

29 CFR 1926.602(c)(1)(viii)(A):   The safety platform was not firmly secured to the lifting carriage and/or forks.    On about December 04, 2019, at the jobsite located at 210 Butternut Avenue, the employer did not ensure that a properly designed work platform was provided that could be safely secured to the lifting carriage/forks of the Lull Model 644B-42, Rough Terrain Telehandler Forklift, Serial Number 00Z24N22-1329, where the equipment was used to elevate personnel.
