A safe distance was not maintained approximately three truck lengths from the truck ahead, and the truck was not kept under control at all times. a. DSC Logistics located in Toledo, Ohio: The employer did not ensure employees operating forklifts in the warehouse were maintaining a proper distance of three truck lengths between forklifts. On November 15, 2019 an employee was driving a Hyundai stand-up forklift in the warehouse and struck another forklift because a distance of at least 3 forklift lengths was not maintained between the two vehicles.
