29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Shop - On or about 12/05/19 and at times prior thereto employees performed maintenance/repairs of equipment including (but not limited to) vertical and horizontal lathes, milling machines, shears, press brakes, and laser cutters. The employer did not develop, implement, and utilize specific lockout/tagout procedures to prevent exposure to moving/rotating parts, and electrical energy. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Shop - On or about 12/05/19 and at times prior thereto the Prime Line Durmazlar 330 Ton press brake S/N 7083981764 was not guarded. b) Shop - On or about 12/05/19 and at times prior thereto the front guard for the 5/8 in. capacity Cincinnati shear #37265 presented a 1 and 3/8 in. vertical gap, exposing operators to the clamps and the blade. c) Shop - On or about 12/05/19 and at times prior thereto the back of the 5/8 in. capacity Cincinnati shear #37265 was not guarded. d) Shop - On or about 12/05/19 and at times prior thereto the 52 in. King vertical lathe was not guarded. e) Shop - On or about 12/05/19 and at times prior thereto the 72 in.
