29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about November 20, 2019 and times prior thereto, an employee installing an Empire fireplace vent on the northeast side of a metal rooftop did not have any type of fall protection at approximately 18 feet above ground level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about November 26, 2019 and times prior thereto, the employer failed to provide training to employees exposed to fall from a residential rooftop when installing Empire fireplace vents.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    On or about November 20, 2019 at about 2:45pm, the employer did not report an inpatient hospitalization that resulted from a workplace fall incident within 24 hours. The employer reported the incident on November 22, 2019 at about 2:53pm, which was past the 24 hours due time to report an inpatient hospitalization.
